The Membership Card is a Fillable PDF.

If you have Adobe Acrobat...

1. Download the form onto your computer.
2. Type your information directly into the boxes and electronically sign your name.
3. Make sure ALL fields are filled out.
4. Save the PDF onto your computer in the following format: Last name-First name-MembershipForm.pdf
5. Email BOTH PAGES of the completed form to: staffrepservices@cwa1180.org

If you do NOT have Adobe Acrobat...

1. Fill out the PDF online when it opens, but some fields may not work.
2. Fill out what you can and then print out the form in order to finish completing by hand.
3. Be sure all fields are complete, sign, and date.
4. Save the PDF onto your computer in the following format: Last name-First name-MembershipForm.pdf
5. Either scan the completed form and email BOTH PAGES to: staffrepservices@cwa1180.org OR
6. Mail BOTH PAGES of a completed copy to: Staff Rep Services, CWA Local 1180, 6 Harrison St., New York, NY 10013
